Care Coordinator - Forensics
Job description
Join LSS and help individuals involved in the criminal justice system achieve recovery, stability, and successful community integration. As a Forensic Care Coordinator, you'll provide person-centered case management for adults enrolled in Wisconsin's Outpatient and Jail-Based Competency Restoration Programs. You'll conduct assessments, develop service plans, coordinate behavioral health and community services, and collaborate with courts, jails, providers, families, and natural supports.
Utilize Motivational Interviewing and trauma-informed practices to support client success. Requires a Bachelor's degree in a human services-related field, valid WI driver's license, and reliable transportation.
