Social Worker Join One of Northeast Florida's Premier Senior Care Communities River Garden Hebrew Home, an award-winning, not-for-profit, five-star nursing and rehabilitation facility located in Jacksonville's beautiful Mandarin area, is seeking an experienced and dynamic Social Worker to join our Social Services Department.

As a Social Worker in our facility, you'll play a key role in helping residents and their families navigate care transitions, emotional challenges, and support services. This is a unique opportunity to make a meaningful difference in the lives of both short-term rehab patients and those requiring long-term support. What You’ll Do: Provide emotional support and counseling to residents and families Coordinate care plans in collaboration with our interdisciplinary team Advocate for resident needs and preferences Ensure documentation complies with regulatory standards What We’re Looking For: A caring, empathetic personality with a genuine desire to help others Strong communication and interpersonal skills Ability to handle sensitive situations with professionalism and compassion Bachelor’s or Master’s degree in Social Work (BSW or MSW) 2-3 years experience in a nursing home or healthcare setting preferred Why Join Us?
