The Advanced Practice Paramedic Mobile Integrated Health (MIH) provides proactive care that is intended to help improve the overall health and wellness of community and program participants. The Advanced Practice Paramedic MIH initiates activities to reduce high volume utilizers through the investigation of and coordination with community partners to address chronic patient issues, conducts home visits to reduce hospital admissions as part of transitional care in conjunction with community partners, encourages and facilitates use of health care services more appropriate to the needs of the patient than the hospital emergency department, particularly for those patients who are in acute crisis...
