Care Coordinator

The Care Coordinator is an integral part of the hospice interdisciplinary team and as such is responsible to the Business Office Manager and/or Administrator for all aspects of the position and role consistent with policies and procedures. The CC is responsible for the completion, storing and retrieving of all medical records.

The CC is also responsible for accurate completion and maintenance of workflow tasks within the electronic health records system.
