Nurse Practitioner or Physician Assistant-PACE

We are seeking a compassionate, dedicated, and team-oriented Nurse Practitioner OR Physician Assistant to serve as a Primary Care Provider (PCP) at our PACE location. The Advanced Practice Provider will deliver participant-centered care within an interdisciplinary team, directly contributing to the health outcomes and quality of life of our participants.

This role requires a combination of direct patient care, team support, and clinical administrative responsibilities. Schedule: Monday-Friday 8am-5pm.
