Responsibilities

of the Case Manager Develops an Individual Rehabilitation Plan (Treatment Plan) with the person served to identify case management needs in the areas of advocacy, coordination of services and resource procurement. Documents communication and interactions with the person served in progress notes in a timely manner. Assures that the assigned cases and all documentation are in compliance and accurate according to agency, COMAR, and CARF standards.

Schedules and conducts home visits with persons served to monitor their condition and to assess their needs at least twice per month. Provides linkages and access to the necessary resources, entitlements and supports to ensure successful community integration.

Qualifications

of the Case Manager A Bachelor's Degree in Social Work, Psychology, Rehabilitation Counseling, or a related discipline is required. Eligible for health care (single and family) the 1st of the month following 30 days of employment. Must work a minimum of 20 hours per week to be eligible for benefits and leave accruals. 403(b) retirement plan with employer match available. This is a full-time (40 hours/week) position.
