49/mile for distance covered between patient home appointments Unit: Home Health Shifts available: Days Shift timings: 8 Hours; Days, 8:00 am - 5:00 pm Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, No weekend or On-call Position Requirements: Registered Nurse: 2 years

• Required Home Health RN experience: 1 year within the last 3 years
• Required EMR: Home Care Home Base (HCHB) charting Must have valid Driver’s License and Personal Auto Insurance Education, License and Certification Requirements: ASN or an equivalent – Required BSN or an equivalent – Highly preferred RN License for State of the state applying or Compact if applicable
• Required BLS Certification from AHA or American Red Cross
• Required Job Description: Provide and coordinate care for assigned home health patients Develop and implement nursing care plans for all all types of visits including Start of Care, ROC, Revisits, etc.,. Support patient recovery. Provide clinical oversight to interdisciplinary Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure. Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
