Case Manager - Step Down Services

Join SSTAR’s Fall River Inpatient Team as a Case Manager for Step Down Services (32hrs/week) Salary Range: $20 -$22/hr Are you passionate about helping individuals build a path to lasting recovery? SSTAR’s Fall River Inpatient Program is seeking a dedicated and motivated Case Manager to join our team! In this rewarding role, you’ll play a vital part in supporting patients as they transition from inpatient care to continued treatment and community resources.

You’ll work closely with a collaborative, compassionate team to ensure each individual has the tools and connections they need for success beyond our program. Education

High school diploma or GED required; Bachelor’s degree in Social Work or a related field preferred. Strong verbal and written communication skills. Open to working with people from diverse backgrounds, cultures and lifestyles in a non-judgmental manner. Direct care positions in addiction programs require two years of continuous sobriety for individuals in recovery.
