We are looking for a Home Health Occupational Therapist (OT) to join our team PRN and help our patients remain comfortable, at home!

This is a great opportunity for an Occupational Therapist with home health experience who loves the life-changing impact your work has on your patients!

Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans

• Provide occupational therapy services in the home setting

• Educate patients and caregivers on adaptive techniques and safety strategies

• Collaborate with physicians, nurses, therapists, and interdisciplinary team members

• Complete timely and accurate documentation

• Monitor patient progress and adjust treatment plans as needed

Qualifications

• Graduate of an accredited Occupational Therapy program

• Current OT license in the state of Ohio

• Home health experience preferred, but not required

• Strong communication and organizational skills

• Valid driver's license and reliable transportation
