Job description
The Physician provides comprehensive care for frail older adults within the PACE Interdisciplinary Team and actively serves on the Center Leadership Team. The role combines clinical expertise with ownership of participant outcomes, center results and a positive culture, supporting participants’ goals, independence and safe community living. Essential Responsibilities:
- Provides geriatric primary care for complex illness, frailty, cognition, function, falls and prevention; completes initial, periodic and annual examinations and interdisciplinary care plans. Consistently prioritizes medication optimization and deprescribing with pharmacy and the team, guided by participant goals.
- Provides timely urgent assessment, treatment and follow-up in the clinic, home or nursing facility; uses telehealth appropriately, arranges emergency transfer when indicated and closes care-transition and referral loops.
- Performs home visits and required or clinically indicated nursing-home visits; participates in hospital rounding, on-call coverage, post-hospital follow-up and medication reconciliation.
- Integrates palliative care through symptom management, serious-illness communication, advance-care planning and goals-concordant treatment; supports caregivers and coordinates hospice or specialty consultation.
- Owns identification and follow-through of behavioral health needs within scope; coordinates care with behavioral health clinicians and the team, including safety assessment and escalation.
- Provides culturally responsive education to participants, families, caregivers and team members; uses qualified interpreters when needed and encourages participant autonomy.
- Actively participates in the Center Leadership Team, Morning Meeting and interdisciplinary reviews; improves workflows, removes barriers, coaches colleagues and follows through within established reporting relationships.
- Shares ownership of reducing falls and avoidable emergency department, inpatient and skilled nursing facility utilization. Applies population health and value-based care principles to prioritize risk, close care gaps and evaluate improvements in quality, safety, access and participant experience.
- Builds a respectful, accountable culture; invites feedback and mediates center conflict through clear roles, agreed actions and follow-through. Escalates unresolved issues and safety, misconduct or HR concerns promptly.
- Adheres to PACE standards and regulations; maintains clinical model fidelity by following Element Care pathways, policies and the NP/MD Collaborative Practice Guide and addressing gaps with the team. Completes accurate, timely documentation, orders, results review and communication, onsite and offsite; protects confidentiality.
- Uses resources responsibly within established budgets and considers total cost of care while maintaining quality and timely access to necessary services.
- Travels locally and performs other duties as required, consistent with the physician role. Job Specification:
- Completed medical residency required; Internal Medicine, Family Medicine or Emergency Medicine preferred. Board certification or approved board eligibility for recent graduates required.
- Current Massachusetts medical license, Massachusetts controlled-substance and DEA authority; appropriate hospital and long-term-care credentials and privileges; current CPR certification.
- At least one year of geriatric care experience; ability to anticipate participant needs, manage complex illness and promote independence.
- Demonstrated teamwork, leadership, clear communication and effective problem-solving; attention to detail, initiative and reliable completion of assignments.
- Ability to work effectively with racially, ethnically and culturally diverse, low-income and special-needs populations; at least one year with low-income and diverse communities preferred.
- Preferred: geriatrics or palliative fellowship or board certification; PACE, population health or value-based care experience; formal clinical leadership; urgent care and integrated behavioral health experience.
- Preferred: clinical proficiency in a PACE-relevant language, such as Spanish, Khmer, Mandarin or Cantonese.
- Advanced clinical technology skills valued.
