CCBHC Triage Specialist

The CCBHC Triage Specialist is responsible for conducting rapid clinical triage, comprehensive biopsychosocial assessments, and initial crisis stabilization for individuals across the lifespan within a Certified Community Behavioral Health Clinic (CCBHC). You will ensure seamless access to care by identifying clinical urgency, determining medical necessity, and initiating person-centered treatment planning using Illinois-mandated tools.

A CCBHC provides a full range of mental health and substance use services for children, adults, and families. The model focuses on coordinated, person-centered care and strong partnerships to help clients access the right services at the right time.
