Social Worker LMSW

Position Summary

The Social Worker Is responsible for providing comprehensive psychosocial#assessment, intervention, resource coordination and discharge planning services#to patients and families. Collaborate with case managers, physicians, nurses,#therapists, community agency members, and other interdisciplinary team#members to address barriers to safe and appropriate discharge and promote#continuity of care.

Education

1. Master#s degree in social work required
2. Texas license preferred. Training and Experience:#
3. A minimum of one-year experience in acute care setting is preferred.
4. Excellent verbal and written skills, well-rounded critical thinking skills,#self-motivated, flexible, and goal-oriented.#
5. Ability to tolerate frequent interruptions and demanding workload. Job Knowledge:#
6. Demonstrates knowledge of the principles of growth and development#over the life span and possesses ability to assess data reflective of the#patient#s status and interprets the appropriate information needed to#identify each patient#s requirements relative to his/her age specific#needs.# Unusual Physical Demands and Working Conditions: Ability to perform:
7. Must be able to move about hospital without major constraints.
