Position Overview

Ohio’s Hospice is seeking a compassionate and dedicated Registered Nurse (RN) to provide high-quality hospice care to patients in Lancaster and surrounding areas. In this role, you will assess patient needs, develop and implement individualized plans of care, and evaluate patient outcomes. You will collaborate with patients, families, physicians, and interdisciplinary team members to ensure care is delivered in accordance with professional nursing standards and the mission of Ohio’s Hospice.

Schedule Employment Type: Full-time Hours: 40 hours per week Regular Schedule: 8:00 a.m. – 4:30 p.m. Additional Requirements: Occasional holiday and weekend rotation Service Area: Lancaster and surrounding communities Key Responsibilities Assess patients’ physical, emotional, psychosocial, and spiritual needs.

Develop, implement, and evaluate individualized nursing care plans. Deliver nursing interventions that promote comfort, dignity, and quality of life. Monitor patient conditions and communicate changes to physicians and the interdisciplinary care team. Collaborate with patients, families, and other healthcare professionals to coordinate care.

Provide education, guidance, and support to patients, families, and caregivers. Maintain accurate and timely documentation of nursing assessments, interventions, and patient care in the electronic health record. Follow professional nursing standards, organizational policies, and applicable clinical requirements. Participate in care planning, team meetings, and quality improvement initiatives.

Support the mission of Ohio’s Hospice by delivering compassionate, patient-centered care.

Qualifications

Graduate of an accredited nursing program; Associate of Science in Nursing (ASN) required, with a Bachelor of Science in Nursing (BSN) preferred. Current Ohio Registered Nurse (RN) license in good standing. Two years of acute care nursing experience preferred. Certified Hospice and Palliative Nurse (CHPN) certification preferred.

Current CPR certification. Computer skills sufficient to maintain accurate and complete electronic clinical documentation. Valid driver's license and a safe driving record. Automobile insurance meeting applicable state minimum requirements. Ability to travel throughout the service area as needed, including during daytime, nighttime, and inclement weather conditions.
