• Employee Assistance Program (EAP) Job Summary: As a Physical Therapist, you will provide skilled in-home therapy services based on individualized treatment plans, working collaboratively with physicians and interdisciplinary team members to maximize patient independence, safety, and quality of life.

What You'll Do

• Complete initial patient evaluations within 48 hours of referral and conduct re-evaluations every 30 days to ensure the plan of care remains appropriate.
• Develop, implement, and modify individualized treatment plans in collaboration with physicians and interdisciplinary team members.
• Maintain timely and accurate clinical documentation, including evaluation reports, daily notes, progress updates, and discharge summaries.
• Educate patients and caregivers on safe mobility techniques, adaptive equipment use, fall prevention, and home exercise programs.
• Communicate effectively with patients, families, physicians, and agency staff to support coordinated, high-quality care.
• Participate in case conferences, chart audits, and quality improvement initiatives to ensure regulatory compliance and positive patient outcomes.

Qualifications

• Current Missouri Physical Therapist License required.
• Current Kansas Physical Therapist License required
• Graduate of an accredited Physical Therapy program (APTA or AMA approved).
• Home Health experience preferred.
• Minimum of two years of clinical experience preferred.
• Current CPR certification required.
• Valid driver's license, current auto insurance, and reliable transportation.
• Demonstrated ability to work independently while maintaining strong communication with the care team.
