• Meaningful, one-on-one patient care in the home setting Job Summary: As a PRN Physical Therapist, you will provide skilled therapy services to home health patients based on individualized treatment plans. You will collaborate with physicians, nurses, therapists, and other members of the care team to help patients achieve their rehabilitation goals and maximize functional independence.

What You'll Do

• Perform re-evaluations and modify treatment plans as appropriate.
• Provide skilled therapeutic interventions to improve strength, mobility, balance, endurance, and safety.
• Educate patients and caregivers regarding home exercise programs, adaptive equipment, fall prevention, and safe mobility techniques.
• Maintain timely and accurate clinical documentation in accordance with agency, state, and federal requirements.
• Collaborate with physicians and interdisciplinary team members to coordinate patient care.

Qualifications

• Current Missouri Physical Therapist License required.
• Current Kansas Physical Therapist License required.
• Graduate of an accredited Physical Therapy program (APTA or AMA approved).
• Home Health experience preferred.
• Current CPR certification required.
• Valid driver's license, current auto insurance, and reliable transportation.
• Strong assessment, communication, and documentation skills.
• Ability to work independently while maintaining effective collaboration with the care team.
