Intake Nurse

About the Job

Caring Hearts Homecare is looking to hire an Intake Nurse to assist in the intake process and assist in day-to-day supervision of direct patient care.

Qualifications

Must have a current RN/LPN license in Massachusetts. Background in public health, home health, or hospital work. At least one year of clinical experience. Hands-on nursing experience. Home care experience preferred. Ability to work independently and manage time effectively. Strong interpersonal skills. Good computer skills.

Responsibilities

Assist in taking referrals and assigning patients to field staff. Follows up on referrals by contacting patients, physicians, family members, etc. Assist in handling patient care supervision and day-to-day patient care management. Assist in managing the HR needs of the clinical department, notifying the Branch Manager/Administrator when new hires are needed to stay proactive with the growth plan.

Assists with new hire screenings/ interviews of home health care personnel. Orients professional staff to clinical policies and procedures of our Agency. Performs other tasks as assigned.
