THE OPPORTUNITY Faith Healthcare is looking for a Clinical Coordinator (RN) to support our Director of Nursing and help lead daily clinical operations from our Miramar office. You’ll coach field clinicians, review documentation, help coordinate care, and conduct nursing visits when needed. This is a hands-on leadership role for someone who can see the bigger picture and still catch the detail that changes it. You should be comfortable leading a conversation, reviewing an OASIS assessment, and heading into the field. Sometimes all three will be on the same day. YOU WILL OWN Supporting the people delivering care. Supervise, mentor, and coach nurses and clinical staff. Be an accessible resource when a visit gets complicated or a clinician needs guidance. Staying close to patient care. Conduct field nursing visits, provide bedside coaching, and help clinicians put good practice into action. Getting the documentation right. Review assessments, plans of care, resumptions of care, recertifications, and discharges for accuracy, completeness, and timely submission. Keeping clinical operations moving. Partner with the Director of Nursing on staffing, case assignments, scheduling, visit utilization, and coordination across disciplines. Following through on concerns. Help address hospitalizations, missed visits, patient complaints, incidents, and changes in patient status. Recognize what needs immediate attention and escalate appropriately. Building a stronger clinical team. Support onboarding, orientation, training, and competency evaluations. Give feedback that is clear, practical, and respectful. Making quality part of the daily work. Participate in chart audits, infection control, Quality Assurance and Performance Improvement (QAPI), survey preparation, and responses to Additional Documentation Requests (ADRs). Working across departments. Coordinate with intake, billing, HR, and operations so clinical needs and operational decisions stay connected. You know when to coach, when to step in, and when to involve your supervisor You also know that “the note is finished” and “the note is accurate” are two different accomplishments. At least two years of home health clinical experience, with current, hands-on field nursing experience. Experience with OASIS documentation, Medicare home health requirements, and clinical compliance. Strong clinical judgment and the ability to explain your reasoning clearly. Comfort with EMR systems, clinical documentation tools, and everyday office technology. The organization to manage competing priorities and the judgment to know which one cannot wait. Especially helpful: Supervisory experience in a Medicare-certified home health agency, familiarity with AI-supported clinical documentation and OASIS tools, and bilingual English/Spanish or English/Creole skills
