Clinical Care Coordinator - Meadowview Recovery Residence - Brattleboro (32 hours per week)

HCRS is looking for a mental health professional to fill a unique opportunity as a Clinical Care Coordinator, providing therapeutic care for adults in a small (six bed), homelike mental health residential setting. The Care Coordinator is responsible for reviewing referrals, engaging with prospective residents, completing assessments and care plans, providing individual therapy and guiding Residential staff in the use of individualized therapeutic interventions.

The Care Coordinator works closely with the Program Coordinator, Residential Nurse and other leadership team members. Master's degree required, with VT licensure or eligible for licensure. This position is for 32 hours per week. The pay for this position is between $49,424 and $52,829.50 annually.
