Care Navigator Case Manager - Adult Services - Brattleboro

Fulltime position in the Brattleboro area as a Care Navigator Case Manager. This role leads service planning and care coordination navigation for Adults receiving mental health services, including managing referrals, providing primary care screening and monitoring and serving as a primary liaison with community partners.

Our ideal candidate will have strong collaboration, problem-solving, and interpersonal skills. Bachelor’s preferred plus 2 years of relevant experience. The pay for this position is $44,850-$58,908.9150 annually.
