Case Manager - Substance Abuse Court (CTI)

The Case Manager is responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered. The Case Manager will manage a caseload of clients and work to ensure they receive the necessary services and support to achieve optimal health and well-being. Services can take place in the individuals home, community, or in a facility.

This role involves assessing clients' needs, developing care plans, coordinating services, and advocating for clients' rights and needs.
